The Midway School Committee made the decision not to increase tuition rates for the upcoming school year due to the current economic situation.
- Include an original, certified copy of the student’s birth certificate. Students must be 4 by September 1 for entry into K-4 and 5 by September 1 for entry into K-5.
- Include a copy of the student’s up-to-date Immunization Record – Ga. State Form 3231. **
- MCCS Student Health Form
- Covenant Contract (Tuition/Fee schedule form) signed and witnessed. Please circle payment choice. (1-pay plan or 10-pay plan.)
- Include a non-refundable application fee of $125.00
** These forms are required by Georgia Law and must be on file for each student. See your pediatrician or health department. All students entering 6th grade and above need to have had a second MMR and Varicella vaccination before admittance. Please send these documents, along with the student application and the registration fee to the school office at the following address: Midway Covenant Christian School, 4635 Dallas Hwy., Powder Springs, GA, 30127.
